Short description of the project idea/activities |
Brief Project Justification The program area is often confronted with situations where close relatives of outpatient nursing care at home are not ready to recognize a disaster or a devastated life time and have lack of the elemental skills of taking care of their relatives. For this reason, such individuals experience significant negative emotions, become vulnerable members of the society, and even with the help of specialists, they are no longer able to properly take care of a sick close relative. High expenses of the government are required for the care of such patients. It is possible to create a support model for such individuals through the use of networking tools with the help of proper settling down of the use of existing resources and a new attitude. There would be united in a network certain operating organizations in a certain area in order to solve the problem (Polyclinics, Hospitals, Social Service Centers, Psychological Help Centers, Non-governmental Organizations, Municipal Institutions, etc.) that can help the vulnerable individuals to solve their problems. An interconnected axis for the entire network would be the establishment of a new training center for those individuals that have outpatients on home care (hereinafter referred to as AMC), which would target such people to the necessary care facilities and at the same time bring them to the center for the various measures of exposure (trainings, seminars, workshops, psychological care etc.). The goal of the project is to create a pilot model for Effective networking impact for outpatients who are having nursing care at home after combining the specific problems of the program area, the good practice of Belarus, Latvia and Lithuania, the available resources and knowledge. The specific objective is the creation and (or) improvement of existing social and other services for vulnerable groups through cross-border cooperation. Project tasks: 1) To investigate the composition, availability and impact of the necessary services on people who are closely related to outpatient care at home, and to describe processes and elements according to partners experience in order to develop and provide a publicly available pilot model for Effective networking impact. 2) To adapt a pilot model in the territories of Kaisiadorys and Partners districts and spread it to the target area of the program. 3) To establish monitoring services and quality assurance elements for institutions operating in the program area. Planned activities of the project: 1) Creation of pilot model for Effective networking impact for people who are close to outpatients that are on their home care. 2) The establishment and the provision of pilot AMC in Lithuania and Belarus by the necessary methodological equipment. 3) Creation and implementation of a service monitoring system in the territory of the program in the dynamic online mode. 4) Organization of internship training courses, seminars, forums for specialists providing outpatient nursing care at home (including patients' relatives). 5) Trainings and practical classes for the target groups. 6) International exchange of practical skills of specialists. |
